Insurance Information

Insurance coverage is one of the first things the admissions team verifies. Having your insurance card available, or knowing your plan name and member ID, allows the team to confirm coverage and handle prior authorization before or on the day of admission.

We accept most major commercial insurance plans and most Medicaid plans. A full-time insurance coordinator handles verification and prior authorization so you do not have to navigate the process alone. A Valid Photo ID

A government-issued photo ID, such as a driver's license, state ID, or passport, is required for admission. This is standard across all licensed treatment facilities and is part of the intake documentation process.

Medical History and Current Medications

Providing an accurate medical history allows the clinical team to plan your detox and treatment safely from the moment you arrive. This includes any current prescription medications, known allergies, previous surgeries or hospitalizations, and any diagnosed medical conditions.

If you are currently taking medications, bring the original prescription bottles. This helps the nursing team verify dosages and manage any medications that need to be continued, adjusted, or tapered during treatment.

Substance Use History

You will be asked about your substance use history during the intake process. Being as specific and honest as possible allows the medical team to assess your withdrawal risk accurately. This includes which substances you use, how long you have been using them, how much and how often, and when you last used.

This information is protected under HIPAA and all applicable federal confidentiality laws. Nothing you share is used against you. It is used to keep you safe during detox and to build a treatment plan that reflects your actual clinical needs.

Mental Health History

If you have a prior diagnosis of depression, anxiety, PTSD, bipolar disorder, or any other mental health condition, having that information available at admission helps the clinical team plan your care more accurately.

Every patient at our facility receives a psychiatric evaluation within 24 hours of admission regardless of whether a prior diagnosis exists. Dual diagnosis, meaning the co-occurrence of a substance use disorder (SUD) and a mental health condition, is common among people entering treatment. Identifying it early means both conditions are treated together from the start.

Emergency Contact Information

You will be asked to provide the name and contact information of at least one emergency contact. This person may be contacted if there is a medical concern during your stay, or as part of the discharge and aftercare planning process.

What to Bring for Your Stay

Our facility operates as an electronics-free environment during treatment. Phones and personal computers are securely stored on arrival and returned at discharge. Patients have access to a personal iPad with recovery resources throughout their stay.

What If You Do Not Have Everything

Missing documents do not automatically prevent admission. If you do not have insurance, if you cannot locate your ID, or if you are unsure about your medical history, call the intake team before arrival. We can work through what is available and determine the best path forward. The goal is to get you into care, not to create additional barriers.
